This agreement must be completed when applying for personal ministry though VMTC.

I am a Christian requesting Christian prayer ministry from VMTC and its personnel.

I have read the Terms & Conditions and I fully understand the nature of the Christian prayer ministry being offered.

I confirm that I am not seeking from VMTC psychological treatment, therapy or professional counselling, and you don't offer any health service and that I will consult a professional if I have any medical or psychological issues.

I acknowledge that I am not asking VMTC to try to change me and I only want you to provide prayer ministry led by the Holy Spirit.

I release VMTC and all its volunteers from all claims or financial compensation now and in the future that I may have as a result of the prayer ministry provided to me by VMTC.

I am 18 years of age or older.
